TOPIC: “Medication Reconciliation in the Elderly”
Assignment Content
The Decision-Making paper serves as a basic template for your final project. This paper will incorporate readings and objectives from all of the modules. It should be written in an APA professional paper format, with a length of 2000 – 3000 words. The length includes all pages (title page, references, and appendices). Each topic should utilize reading assignments, videos, and articles provided as additional material in each module.
There are many resources available to help you with writing. The FHSU Forsyth Library Subject Guides have links to APA resources, and as an online student, you have access to Online Tutoring at FHSU. I strongly encourage you to utilize the FHSU Writing Center as you work on this paper throughout the semester. Also, take advantage of your Grammarly subscription provided by FHSU. A significant part of your grade is based on your writing, including APA format, grammar, and following length requirements. Use your APA Manual!! Ask questions if you are unsure.
Project Topic
This project may be hypothetical, aimed at the community, clinic, or hospital. You will continue to build on your idea as you advance in the program. Discuss the project topic, including the importance of the project and evidence to support it.
BSN to DNP Students: Select a topic that a family nurse practitioner could implement as a project. You will continue to refine your project topics throughout the program.
Considerations
Legal and Ethical
- What ethical considerations will you keep in mind for your project? (examples: Fairness, Integrity, Respect for Stakeholders)
- Does your facility have a department that works with student projects?
- How can you ensure that the project complies with relevant laws and regulations?
- How can you protect sensitive information and respect privacy laws?
Political and Environmental
- How will you navigate and comply with existing policies and regulations? (This can be at various levels, such as local, state, national, or international.)
- How will the political stability of your organization influence your decision-making? (examples: changes in administration, policy shifts)
- How will your decisions affect the community and the environment?
Social and Cultural
- What is the practice climate for quality improvement projects?
- Is there support and advocacy for the project, including time and resources?
- How will your project impact employees?
- How can you ensure effective communication?
Economics and Sustainability
- Does your project have sustainability? How can you ensure the project’s long-term economic viability?
- How can you identify and mitigate economic risks? (financial, operations, etc.)
- Will the project require resources, such as supplies and labor?
- How will your project benefit your organization? Community?
Technology
- Will you work with IT to update electronic health record documentation?
- What technology will the project require?
Collaborators
Interprofessional collaboration is an essential feature of effective QI Teams. Although not traditionally included in the team, engagement of patients, family members, and consumers as team members is becoming more common as patient-centered, whole-person care becomes the norm

Gantt Chart
- Present the project timeline as a Gantt Chart, including significant data points, such as:
- October – December: Identify the Project Team and Complete the Literature Review
- January – February: Determine project methodology
- January – March: Write Project Proposal
- March: Present Project Proposal to DNP Faculty and Classmates
- April: Submit IRB application and Project Proposal Paper
- August – December: Implement Project
- January – March: Analyze data and complete the Final Project Paper
- April – May: Disseminate Project Results
- Insert the chart as a figure or appendix, and discuss it in the paper.
